WHAT TO EXPECT

Preparing For Your Appointment

DENTAL CONSULTATION

Your initial appointment may consist of a consultation explaining your diagnosis and treatment options. Occasionally, procedures can be performed the same day as the consultation; however, especially with a complex medical history or treatment plan, a dental exam and a second appointment may be recommended for additional consultation and treatment.

Please assist us by providing the following information at the time of your consultation:

• If you have dental insurance, please bring any pertinent information and a valid form of identification. This will save time and allow us to help you process any claims.
• A current list of medications you are taking.
• A list of past surgical procedures and any x-rays or images if applicable.

IMPORTANT: All patients under the age of 21 must be accompanied by a parent or guardian at the consultation visit and all subsequent visits.

Please alert the office if you have a medical condition that may be of concern prior to your appointment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, bone density medications, etc.)

X-RAYS AND IMAGES

If your dentist or physician has taken x-rays, you may request that they forward or email them to our office. If there is not enough time, please pick them up and bring them to our office or let us know, and we will try to make arrangements with your referring doctor. In many cases, additional films may be necessary, and they will be taken at our facility.
